The three rules that matter most

01Ascend gradually

Once above 3,000 m, keep increases in sleeping elevation modest and build in acclimatization time.

02Do not ascend with symptoms

If you develop symptoms consistent with altitude illness, do not sleep higher until they have resolved.

03Descend if worsening

Symptoms that worsen despite rest at the same elevation are a reason to descend and seek appropriate care.

What is altitude sickness?

Acute mountain sickness (AMS) can occur after a recent ascent to higher elevation. Headache is common and may occur with nausea, dizziness, fatigue, poor appetite, or vomiting. Risk depends strongly on the altitude where you sleep and how quickly you ascend.

Preparing for higher elevations

Plan the ascent, not just the destination

Gradual ascent gives the body time to acclimatize. Current CDC and Wilderness Medical Society guidance emphasizes avoiding a rapid jump in sleeping altitude. Above roughly 3,000 m, a conservative itinerary keeps increases in sleeping elevation to about 500 m per day and adds acclimatization days as elevation increases.

Fitness is not protection against AMS

The old version of this article suggested that being physically fit lowers altitude-sickness risk. That wording has been removed. Physical fitness can help with hiking performance, but it does not make a traveler immune to altitude illness.

Hydration is useful, but it is not a cure

Drink normally and avoid becoming dehydrated, especially during exertion. But drinking extra water does not replace acclimatization and should not be described as a proven way to prevent AMS.

During the first days at altitude

  • Keep exertion lighter during the first 48 hours at altitude.
  • Avoid alcohol during the first 48 hours.
  • Pay attention to headache, dizziness, nausea, unusual fatigue, confusion, balance problems, or breathing difficulty.
  • Do not continue to a higher sleeping elevation while symptoms of altitude illness are present.
Urgent warning signs: confusion, loss of coordination, severe or worsening breathlessness, or symptoms that continue to worsen at the same elevation can signal a serious altitude illness. Descent and urgent medical evaluation may be necessary.

What about altitude medication?

Acetazolamide can be useful for some travelers, particularly when gradual ascent is not practical or when a traveler is at moderate or high risk. Whether it is appropriate depends on the itinerary and individual medical history, so medication decisions belong with a clinician rather than an affiliate product recommendation.

Does being fit prevent altitude sickness?

No. Fitness can make activity easier, but CDC guidance notes that training and physical fitness do not determine susceptibility to altitude illness.

Does drinking lots of water prevent altitude sickness?
